There when you needed them most

Most days where you wanted it

It to be the last one

One that didn't end then

Then you had love

Love you didn't believe in

In a month it came to you

You were very scared

Scared of commitment

Commitment to being hurt

Hurt that never appeared

Appeared to be the last

Last love you would ever feel

Feel free to fall again

Again comes the fear

Fear of being left

Left hand grows heavy

Heavy from the love

Love you share with them...

I too really like the way you've structured this, starting each line with the end of the last.

I like title, the only way I could see it to be more fitting is that if the very first and last words of the poem were the same; in keeping with your structure and implying the continuous loop.
Just as an example you could tweak it to say "Love you share with those / those who make you feel welcome"
But it's just fine the way you have it too.

Overall I think this poem implies great empathy and understanding. Though there's much more to it. Well done.
