You- it means how the man knows everything about the pleasure of his partner.
Without doing much
Showed me paradise

As the back of your hand- means how the man in the poem knows about all the erogenous point of his partner and he's skillful that it feels almost effortless.
We are in the same song
In each others customised hell.- both the lovers going through issues.
Keeping prisoner means to extract more time from each other to extract more passion.
Bahr el ghazal and sobat - are two tributaries of Nile which means the reunion of the lovers to become one.
