You're not the first to ask about this bit, so i must need to clarify better:
"Get the f**k out

where you can shove your envy.
It isnt my fault you feel guilty,

accidentally." - the narrator doesn't care about anyone and is content that way. the doctors are prying into it as lack of conscience; narrator reads it as envy. Maybe it is envy; i don't know.
Yeah, i'll need to re-examine those lines and see if i can fix the problem.
