it includes the letters of your name, usually done verticall like...

D
with each letter beginning a new line about yourself, something like...
Beating
On
Your
Dog

...but hopefully you can come up with something better than that! you can use first name, middle name, last name, nickname...the more creative the better...

post all poems under the title "BIO POEM: ________________ (your name)"
