It's, sorta like a death note. I'm taking the name, and some lines from Tupacs: In the event of my Demise.

"
In the event of my demise,
when my heart can beat no more.
I hope I die for a principle, 
or a belief that I had lived for.
I will die before my time,
because I feel the shadow of death.
I will die before my time,
because I feel the shadow of death.
So much I wanted to accomplish,
before I reached my death.
I have come to the grips with the possibility
and wiped the last tear from my eyes.
I love all who were positive

In the event of my demise,
remember me not for all the pain I caused.
I know I fucked up,
I know I failed.

In the event of demise,
don't cry.
Remember me for what I wanted to become.
Throw a big party, let em all rock it.
Throw it in my memory.

In the event of my demise,
let there be no funeral.
Cremate me, and lay the ashes down. 
Or keep them, if you want.

In the event of my demise,
let their be no doubt that I'm gone.
No people taking over my s**t.
No one trying to ruin my name.
No one to try to help my name out.
Let me be gone, I'll see you in the next life.

In the event of my demise,
Do not name kids after me.
Do not think of me as epic.
Use me as an example of what not to be.
Do not allow your kids to become as I did.

In the event of my demise,
my life will be incomplete.
So much I wanted to do.
So much I wanted to try.
But it was not the way.
But this is just..
