"You held me" should have a semi colon, not a comma. Also, try adding a line that is different from the rest like, "You knew me before I knew myself, and that's why I love you," or something like that. It'll help is end lovely and gently. The poem is sweet and very touching. This is one of my favorite pieces of yours :)
